Students and Parents/Guardians:
On Wednesday, April 14, Cabin Creek Health Center will be administering Pfizer vaccinations at Riverside High School in the auxiliary gym from 8:30 am to 12:00 pm.
These vaccinations will be for students ages 16 and up and any family members. Students and family members must first contact Cabin Creek Health to arrange a vaccination by emailing ([email protected]) or by calling (304-949-3591); this is required to ensure we have enough vaccinations.
Any student wanting a vaccination must bring a permission slip form signed by a parent or guardian -- (click here to download the form). If a student does not bring a signed permission slip form, he or she will not be able to get a vaccination. Family members scheduled to receive a vaccination will enter Riverside High School through the auxiliary gym exterior doors. Do not come to the main office.
